Yorkshire-based LGBTQ+ charity Out Together has proudly partnered with Patient.info, one of the UK’s most trusted digital health platforms. Together, they have secured joint funding through the Department for Science, Innovation and Technology’s Digital Inclusion Innovation Fund to develop vital digital inclusion hubs and support services for older LGBTQ+ people across West Yorkshire.
With funding support and a strong partnership, the community-led charity and health platform are driving meaningful change to tackle health and social inequalities that can put the older LGBTQ+ community at higher risk of poor health outcomes, financial vulnerability and social isolation. By expanding Out Together’s digital inclusion hubs across West Yorkshire, they aim to improve the overall well-being of hundreds of people in the region.
The hubs aim to empower people to engage with technology on their own terms – to improve their lives, going far beyond simply learning how to use devices. The pilot digital inclusion hub in Leeds has already seen considerable success.

The partnership between Out Together and Patient.info creates a unique opportunity for individuals to access fully funded, tailored support. This aims to make a real difference in breaking down barriers that many older LGBTQ+ people face in different aspects of their lives. Through providing essential online tools, knowledge, and skills support, to help people live independently, confidently, and with connection in today’s digital world. Each hub also provides a welcoming space where people can learn at their own pace, meet others, share experiences, and rebuild social connections that may have been lost through isolation.
The extensive support offered through the hubs includes group training, peer-to-peer learning led by community volunteers, one-to-one support, and access to devices. People accessing support will find tailored packages that meet their individual needs. These range from learning how to email and video call, to using online banking, NHS services, and other essential tools for modern daily life.
Alongside the expansion of the five digital inclusion hubs, which will open in Bradford, Wakefield, Huddersfield, Hebden Bridge, Harrogate and York, Out Together and Patient.info are also developing a national digital health hub and volunteer network. This aims to deliver sustainable, long-term support, allowing more people to access services even if they cannot do so in person.
